Why people faint, and when to investigate

Syncope is the medical term for fainting, or passing out. It is caused by a temporary drop in the amount of blood flowing to the brain.

Syncope can happen if you have a sudden drop in blood pressure, a drop in heart rate, or changes in the amount of blood in areas of your body. If you pass out, you will most likely become conscious and alert again straight away, although you may feel confused for a little while afterwards.

Because a slow heart rate and a rhythm disturbance both sit on that list of causes, fainting is worth investigating properly rather than dismissing as a one off. The workup usually starts with an ECG and a 2D Echo, with a Holter monitor added if the episodes arrive without warning.

Some patterns matter more than others. Fainting during exertion, fainting with no warning at all, or fainting in someone with known heart disease or a family history of sudden death all warrant a prompt cardiac assessment rather than watching and waiting.
