Understanding it

What an ECG and a 2D Echo actually show

An electrocardiogram is a representation of the heart’s electrical activity, recorded from electrodes on the body surface. Electrode patches are attached to your skin to measure the electrical impulses given off by your heart, and the result is a trace that can be read by a doctor.

It can give information about previous heart attacks or problems with the heart rhythm. Normal rhythm is called sinus rhythm, because it originates from the sinus node. Other abnormal rhythms the ECG can identify include atrial fibrillation and the various forms of heart block. An ECG may also provide clues that a chamber in the heart is enlarged.

An echocardiogram, commonly known as a 2D Echo, is simply a sonogram of the heart. It provides detailed information about the structure and function of the heart, including the pumping function and the function of the valves. The technology uses sound waves to create images, so there is no radiation exposure at all.

The squeezing function of the left ventricle, the main pumping chamber, is calculated and called the left ventricular ejection fraction, or LVEF. That figure estimates the percentage of blood ejected from the left ventricle with every heartbeat. All four valves are looked at in close detail to confirm they open and close normally, and any other structural abnormality can be seen and evaluated. Sometimes contrast is needed to visualise the heart better, which involves inserting an intravenous line and injecting a small amount of contrast liquid.

Rhythm and rate

An ECG can show evidence of a heart attack you may have had in the past, even if you had no symptoms at the time.

Chamber enlargement

The trace can give clues that a chamber of the heart is enlarged, which the echo then confirms and measures.

Pumping function

LVEF, the percentage of blood the left ventricle ejects with every beat, is calculated and tracked across visits.

Valve function

All four valves are examined in close detail to confirm they open fully and close tightly.

Structure and contrast

Structural abnormalities are evaluated, and where the images need improving a small amount of contrast is given through an intravenous line.
